Fusion is the process of merging two separate beings into one, combining their attributes, from strength and speed to reflexes, intelligence and wisdom.In the anime,atleast five characters fused.
Goku+Vegeta=Gogeta
Goku+Vegeta=Vegito
Goten+Truns=Gotenks
Kibito+Supreme Kai=Kibito Kai
Piccolo+Kami+Neil=Kamicolo(Commanly called Piccolo)

No that doesn't exist.Goku and Gohan planned to do but Gohan died before it happens.

Yes they die.Goku died for two times.
Vegeta for 1 time.
Gohan for 1 time.
Piccolo for 1 time.
Trunks for 1 time.
Krillian for 0 times.

They ask the Dragon to revive the person back.
Earthian Dragon can grant 1 wish and can recover the person only 1 time back to life.
Namekian Dragon however can grant 3 wishes.
